We pair licensed physicians with Health Advocates to help patients untangle the complexity of U.S. healthcare, overcome social determinant barriers, and make confident, informed decisions. By combining compassionate clinical expertise with hands-on navigation and resource linkage, we give every patient the knowledgeable ally they deserve.

About the Role

This is not a typical social work or nursing role. As a Health Advocate at Careway, you will be the frontline support for Medicare patients navigating a complex healthcare system. Your mission is to remove barriers, including scheduling challenges, interpreting doctor instructions, insurance paperwork, and social determinants like transportation needs.

This role requires grit and resilience. You will frequently navigate complex medical and social situations, and you must be prepared to de-escalate and support patients who may be frustrated, difficult, or overwhelmed by their circumstances. Working closely with our physicians, you will build trusting, long-term relationships and provide consistent support to ensure patients achieve their healthcare goals.

Note: This is a 1099 contract role. To ensure continuity of care for our patients, this position requires a baseline capacity of 20+ hours per week, and we prioritize candidates with 25+ hours of weekly availability. Because much of your work involves coordinating with clinics and insurance companies, you must be available to operate during standard business hours. All patient interactions are performed remotely via video or phone call.

What You’ll Do

  • Guide patients through healthcare challenges, clarifying complex medical information in simple terms, coordinating seamless care, and assisting with insurance paperwork.

  • Manage difficult or complex patient cases with empathy and firm boundaries, ensuring progress even when facing resistance or systemic roadblocks.

  • Create and manage individualized care plans in partnership with Careway physicians, addressing social determinants of health such as transportation, food, home support, and digital access.

  • Proactively identify patient needs, prioritize actions, and coordinate appointments, referrals, and follow-ups.

  • Empower patients by improving their health literacy and self-management skills through education and coaching.

  • Collaborate with our clinical and operational teams, sharing insights to continuously improve Careway’s advocacy-driven care model.

What You’ll Bring

  • Experience in patient advocacy, care coordination, social work, healthcare navigation, or a related field.

  • Deep understanding of Social Determinants of Health (SDOH) and experience working with diverse, often vulnerable patient populations.

  • Exceptional empathy, active listening, and interpersonal skills, enabling you to build trust quickly and maintain meaningful relationships.

  • Strong organizational abilities and clear communication, allowing you to manage multiple tasks effectively.

  • Tech-savvy and capable of quickly learning modern EHR systems, digital collaboration tools, and patient engagement apps.

  • Action-oriented mindset with a commitment to improving patient outcomes and system efficiency.

Applicants must be based in the United States.

Nice-to-Haves

  • Certification in nursing, patient advocacy, social work, or related field.

  • Background in community health, health coaching, or experience with Medicare populations.

  • Fluency in Spanish or other languages.
