- Support care coordination activities, including scheduling follow-up appointments and communicating with members or their caregivers.
- Mail and/or email patient care plans and educational materials as directed by case managers.
- Accurately update and maintain patient records within the electronic health record (EHR) system.
- Protect patient confidentiality and ensure compliance with all HIPAA and organizational policies.
Experience, Skills, & Personality
- Certified or Registered Medical Assistant (CMA/RMA) required.
- 2 years of experience in a medical office, telehealth, or care coordination setting.
- Experience using EHR systems (eClinicalWorks preferred).
- Strong organizational skills and high attention to detail.
- Excellent written and verbal communication abilities.
- Self-motivated and able to work independently in a fast-paced, remote environment.
- Experience working with older adult populations preferred.
- Naturally empathetic, with strong active listening and problem-solving skills.
