Job Description
We are seeking a detail-oriented and compassionate professional with experience in Chronic Care Management (CCM) to support ongoing patient engagement and care coordination. This role focuses on outreach to patients in California, ensuring consistent follow-up, appointment scheduling, and overall patient support.
Key Responsibilities:
- Conduct monthly outbound calls to Medicare patients enrolled in Chronic Care Management programs
- Follow up with patients to monitor health status, address concerns, and ensure continuity of care
- Schedule appointments and coordinate follow-ups as needed
- Maintain accurate and timely documentation of all patient interactions
- Support patient adherence to care plans through consistent communication
- Collaborate with healthcare providers and administrative teams to ensure high-quality patient care
- At least 2 years of experience as a front desk receptionist, patient coordinator, or in a similar role within a U.S.-based healthcare practice.
- Experience with Chronic Care Management (CCM) strongly preferred.
- Bilingual in English and Spanish, with excellent written and verbal communication skills.
- Comfortable being on camera throughout the full 8-hour shift.
Note: Please note this role requires to be on camera the 8 hours shift.
This is a full-time contractor position (40 hours per week) with a pay rate of $7 – $8 per hour depending on skillset and experience. The schedule is Monday through Friday, from 8:30 AM to 5:30PM PST.
